How to Prepare for the Appointment

The best way to prepare for a consultation at the Muzzi Mirza Pancreatic Cancer Prevention & Genetics program is to collect as much information as possible on your family's health history, especially in regard to cancer. If there is a history of cancer in your extended family, knowing exactly what kind of cancer it is as well as the age at which the family member was first diagnosed with the cancer are important pieces of information.

If you or any of your family members have had any biopsies or genetic tests, please bring those results with you to your appointment. If you have had recent imaging studies like an MRI or CT Scan, please bring copies of the images and accompanying reports.

At your appointment you will be given an extensive questionnaire, the Health History Questionnaire, which you will be asked to take home to complete. This questionnaire guides you through the information we will need to have regarding you and your family in order to properly assess your risk. The questionnaire is also a part of the Pancreatic Cancer Prevention Program Registry which is a clinical trial you may be asked to participate in during your initial visit to the Muzzi Mirza Pancreatic Cancer Prevention & Genetics Program. Participation in the trial is completely voluntary and filling out the questionnaire prior to your appointment does not require you to participate in the clinical trial.

Before coming in for your appointment, you may want to spend some time with your family preparing any questions you may have to make the most out of your visit with our specialists.


Make sure to bring:
  • Your INSURANCE CARD
  • BIOPSY AND GENETIC TEST RESULTS for you or your family members
  • RADIOLOGY FILMS, CDs or IMAGING STUDIES previously performed
